Cancellation Policy
Elmhurst Foot & Ankle Center, Inc. is proud of its clinicians, providers, and staff because they are devoted practitioners who are offering excellent care to our patients. To not break the rule, we implemented an exact appointment cancellation policy with its effectiveness from September 1st, 2024.
Cancellation Notice Requirement:
- 24-Hour Notice: Elmhurst Center requests a 24-hour notice to take effect if patients want to cancel their appointment. Patients are required to notify Elmhurst Foot & Ankle Center at least 24 hours before at the time of appointment.
No-Show and Same-Day Cancellation Fees:
- Same-Day Cancellations: A $25.00 fee is used in the same appointment that is being canceled on the same day.
- No-Show Appointments: A $50.00 fee is paid by every appointment holder who misses a session without giving us former notice.
Fee Payment and Future Appointments:
- All must pay all the no-shows and same-day cancellations before any future appointments can be made or kept.
Termination of Services:
Elmhurst Foot & Ankle Center reserves the right to end services if:
- The patient no-shows either 3 or more times by a month or cancels two or more appointments with less than 24-hour notice alone each month.
- A patient misses all of their 3 appointments without prior notification within a 3-month period.
We can relate to the fact that sometimes you might experience situations that are out of your control, and we assure you that you can feel free to contact us in case you have any issues or concerns about this policy. Our team is indeed ready to help you on your way with whatever problems may occur and we are resolute to keep on serving you.
