No Show / Cancellation Policy
At Elmhurst Foot & Ankle Center, Inc., our clinicians, providers, and staff are dedicated to delivering the highest quality care to our patients. To maintain this standard, we have established a strict appointment cancellation policy, effective September 1st, 2024.
Cancellation Notice Requirement:
• 24-Hour Notice: Patients are required to notify Elmhurst Foot & Ankle Center at least 24 hours prior to their scheduled appointment if they need to cancel.
No-Show and Same-Day Cancellation Fees:
• Same-Day Cancellations: A fee of $25.00 will be charged for any appointment that is cancelled on the same day.
• No-Show Appointments: A fee of $50.00 will be charged for any appointment that is missed without prior notice.
Fee Payment and Future Appointments:
• All no-show and same-day cancellation fees must be paid before any future appointments can be scheduled or fulfilled.
Termination of Services:
• Elmhurst Foot & Ankle Center reserves the right to terminate services if:
• More than two (2) no-shows or cancellations with less than 24 hours’ notice occur within a single month.
• A total of three (3) no-shows occur over a span of three consecutive months.
